Job Description:
Summary Coordinates the care needs of assigned patients and
develops comprehensive plans to manage care delivery across the
patient care continuum.
Partners with patients and their primary physicians to develop
customized care plans based on their individual needs and
preferences.
Responsibilities
Preferred: Bachelors or Masters degree Experience Required: 2 years
nursing experience. Certifications Required: Must be currently
licensed, certified or registered to practice profession as
required by law, regulation in state of practice or policy. CPR
Tenet complies with federal, state, and/or local laws regarding
