POSITION SUMMARY Serves the organization by working with customers
to include: inpatients, outpatients, physicians, allied healthcare
professionals and the community by providing services such as
education, referrals, and screenings. Utilizes clinical expertise
in managing diabetes patients by providing appropriate information,
training, and resources while employing effective problem-solving
techniques to achieve desired outcomes. Ensures that he/she is
current with new diabetes treatment technology Your
responsibilities will include: - ESSENTIAL FUNCTIONS: 1. Provides
individual and group education to diabetes patients. 2. Assists
with coordinating diabetes patient care throughout the continuum.
3. Serves as a clinical resource and consultant for issues related
to diabetes. 4. Participates in the marketing of department
personnel and services. 5. Collaborates with all disciplines to
enhance clinical outcomes, reduce length of stay, improve resource
utilization and ensure patient satisfaction. 6. Provides continuing
education to patients, hospital staff and the community. 7.
Participates in complying with regulatory standards and
requirements for maintaining recognition status for the outpatient
program as well as the advanced disease specific care certification
for inpatient diabetes care. 8. Establishes a system for
understanding and communicating effectively with physicians
involved in diabetes care. 9. Maintains a state-of art approach to
diabetes care and self-management training. Other duties as
assigned. What qualifications you will need: - EDUCATION REQUIRED
AND/OR PREFERRED: (list required and preferred separately)
Bachelors of Science Degree (Nursing) with current license or
Bachelors of Science Degree in Food and Nutrition and successful
completion of dietetic internship experience through Academy of
Nutrition and Dietetics program LICENSURE/CERTIFICATION: (list
required and preferred separately) Required: Current Nursing or
Registered Dietitian by Academy of Nutrition and Dietetics,
Licensed by the State of Texas Required: BCLS/AED certification for
"Healthcare Provider" by the American Heart Association PREFERRED
EDUCATION, LICENSURE, CERTIFICATION, EXPERIENCE, SKJLLS KNOWLEDGE
AND ABILITIES: Minimum 5 years of experience Preferred: Masters
Degree /Nursing or Dietetics Preferred: CDCES (Certified Diabetes
Care and Education Specialist) certification within two years. Del
Sol Medical Centeris a full service, acute-care hospital in east El
Paso, Texas. We have a Level II trauma designation. This facility
has 300 patient beds. Our range of services include emergency care,
cardiac care, women’s services, Level III NICU, rehabilitation, a
bariatric clinic, and a Minimally Invasive Surgery Center. Del Sol
