GENERAL DESCRIPTION: Under the direct supervision of the PACE
Center Director, the Social Work Manager has supervisory
responsibilities over the Social Work departments at all centers.
The Social Work Manager will ensure that all Social Work department
service delivery functions and services are provided in adherence
to and compliance with all applicable Federal and State regulations
and requirements governing PACE and in accordance with Bienvivir
policies and procedures. QUALIFICATIONS / REQUIREMENTS: Graduate
from an accredited university with a Master's Degree in Social
Work. Have licensure by the Texas State Board of Social Work
Examiners. Five (5) years of direct service experience in a Social
Work setting. Three to Five (3-5) years of supervisory experience
required. At least one (1) year of experience working with an
elderly population. Have strong computer skills with the ability to
utilize Microsoft Office software applications. Have strong
communication and conflict resolution skills. Be able to promote a
team work environment. Have strong organization and time management
skills. Be bilingual English/Spanish. Be able to lift up to 30
pounds. Be able to prioritize work to fulfill the mission of the
Organization. Job Requirements:RESPONSIBILITIES: Supervise all
Social Workers and staff assigned to the Social Work departments of
all -centers in accordance with their specific job description.
Monitor and assess the individual and collective performance of
Social Work department -personnel to determine compliance with
social work standards of performance, with -Bienvivir Social Work
department policies and procedures, and with Federal and State
-regulations. In particular, the Social Work Manager will ensure
optimum communication -and coordination with the Interdisciplinary
Team in the development and implementation -of comprehensive and
individualized participant care plans that represent the PACE
-approach and philosophy. In coordination with the QAPI Coordinator
and PACE Center Directors, develop -measureable quality improvement
goals and objectives with accompanying performance -tracking
methodologies. Perform probationary and annual evaluations of all
Social Work department personnel and -provide individual guidance
and mentoring for improvement of performance. Also -responsible for
new employee orientation checklists as well as disciplinary action
forms as -required. As a leader and manager, serve as a mentor and
be accessible for consultation and guidance -to department
personnel requesting assistance with challenging cases as they
relate to social -services. Develop, review and/or modify
department policies and procedures to reflect the spirit and
-intent of the philosophy of PACE/Bienvivir and to conform to all
Federal and State -regulations and requirements. - Ensure optimum
coordination and communication with hospital and placement
facilities -case management/social work departments to ensure
appropriate transition to and from -facilities. Work with IDT
members to ensure Social Workers complete assessments and
-comprehensive Plans of Care by scheduled deadlines and in the
manner dictated by -regulatory standards. Play an integral part in
IDT meetings when in attendance to ensure Social Work personnel
-report on participant conditions (or change of condition), work
with the Team to -develop/update Plans of Care, and communicate any
participant and/or family requests, -comments, or concerns.
Participate with assigned participant Social Worker and other IDT
personnel in all Appeals -Hearings and serve as lead presenter,
historian and participant advocate during these -hearings.
Responsible for researching and making recommendations to Senior
Management for -training opportunities that will expand and
complement department personnel skill sets, as -well as any
education programs that may be of value to Bienvivir participants
and/or -participant families. Social Work Manager will be
responsible for scheduling and -coordinating these training
sessions once approved. Ensure department personnel are current
with all licensure, CEU, and Human Resource -requirements as
prescribed by Bienvivir policy and regulatory standards. Conduct
periodic department electronic medical record audits to ensure
medical record -integrity, documentation completion and accuracy,
and institute any corrective action -measures should deficiencies
be found. Participate in any organization committees/work groups as
assigned by Management to -ensure Social Work representation.
Participate in organization's annual budgetary planning.
Responsible for ensuring -department is functioning within budget
parameters and personnel are exercising fiscal -responsibility
while fulfilling their job responsibilities. Responsible for
department staffing to include the management of PTO schedules and
any -long-term absences due to medical leave, ensuring there is
adequate personnel coverage at -all times and that Time Force is
verified and submitted to Payroll Department by scheduled
-deadline. Social Work Manager will develop an On-Call Calendar,
oversee and assign personnel to -the on-call schedule ensuring the
responsibilities of this duty are performed per standard. Social
Work Manager will be expected to be available to their personnel
for consultation at -all times. Work with the Compliance department
and PACE Center Directors to address any grievance -or incident
report investigations as assigned. This position is responsible for
overseeing appropriate and coordinated discharge planning -that
meets the needs of the participant. Participate in Annual Memorial
Committee. Responsible for facilitating Annual Social Work Skills
Fair for Social Work department in -an effort to standardize
operations, ensure competencies are up to date, and train
personnel. The Social Work Manager will monitor and assess the
individual and collective -performance of the Social Work
department personnel on a weekly basis in the Care -Planning
Meetings to determine compliance with social work standards of
performance. Work with State MEPD to coordinate annual financial
recertification of participants; -assisting participants/families
through the recertification process. Responsible and accountable
for the development of competency, skills, and ethical -practice.
Implement policies that enable the Social Work department to
provide effective services. May need to conduct home visits. During
these visits, possible work condition exposure to -but not limited
to, unsanitary/poor living settings, pets, and participants with
behavioral or -psychological health conditions. All other duties as
assigned by the PACE Center Director and Senior Management.
